There are two ways to make an appointment - either contact me by phone or email or fill out the electronic client intake form that is available here. You can fill it out on your computer and then email it by simply pressing the "Submit by email" button on the form, or print it and mail it to me.

Note that you will need Adobe Reader. If you do not have that free software installed on your computer, you can get it by clicking the link below, which will take you to Adobe Systems' web site. Follow the detailed instructions on the Adobe web site to download and then install the software on your computer.
